Are the extra bucks only good on that same item? For example, if you buy the Colgate, will the extra bucks say “good on your next purchase of Colgate”…?

I have a question how does the extra bucks work, do you get the item for free or do you pay for the item and get credit for a future order?

You get the extra bucks at the end of the transaction. It does not go towards that purchase.

You have to buy the item and you will get the extra bucks on the bottom of your receipt for a future purchase.

I am going to have to break up my trip so i can use my EB i have and then the new one in the second transaction so i dont have to pay a lot out of pocket . I told my family since i dont have money i will be making gift baskets for Evey one .

Extra Care Bucks that you earn are good on your next purchase. so, when you get your receipt, theyll print out, and you can use that credit on a FUTURE purchase. IF you are doing a 2nd transaction, you can use them then, too. You stilll will pay for whatever item(s) that you have, and earn the ECB (extra care bucks) in some cases, in the same amount, so that is what makes them free. You can combine them, so if you have an ECB of $1, and $1.99, $5 and so forth you can use all of them together.

When you checkout, if you have a CVS coupon for say, $4 off of $20, give that to the cashier first, THEN any other CVS coupons that you have, if any, (like those that print at their coupon Kiosk or come in the paper), THEN, give your manufacturer coupons (if any), THEN your Extra Care Bucks. That is how I do it anyhow. :)
